POSITION SUMMARY - RN Case Manager

POSITION DUTIES - Care Manager for discharge planning and care coordination, requires prior acute care discharge planning experience

MINIMUM REQUIRED QUALIFICATIONS - 2-4 years RN CM in acute care facility is required; RN license

LENGTH OF ASSIGNMENT - 13 weeks

SHIFT / HOURS PER WEEK - 8 am – 4:30 pm / 40

SYSTEMS - Meditech, Midas

START DATE - 12/16/2024
