POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS – RN license; Bedside patient care experience; 2 years hospital CM experience or MSW
PREFERRED QUALIFICATIONS – ACM or CCM
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 7a-7p
SYSTEMS – Meditech / Midas
START DATE – 12/16/2024
