POSITION SUMMARY - RN Case Manager
POSITION DUTIES - Care Coordinator (case manager) nurse positions to collaborate with Nursing and Providers to support patient care within our Nahata'Dziil Clinic in Sanders, AZ.
MINIMUM REQUIRED QUALIFICATIONS - Minimum Years of Experience:5
LENGTH OF ASSIGNMENT - 13 Weeks
SHIFT / HOURS PER WEEK - Weekdays, Monday through Friday from 8a to 5p (no weekends or holidays).
