POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS – RN license; BLS; 2 years current experience in specialty; Inpatient/Acute Care experience (minimum, current 12-months); Epic experience; At least one 13-week previous travel assignment.
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – Varied with 8am start / 40hrs per week
SYSTEMS – Epic
