RN Case Manager

Responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating physical and psychological factors, financial, social, and community resources in providing client services. From pre-admission to post discharge.

Educational and/or Professional Qualifications:
Bachelors or Associates degree in Nursing.
Previous Experience:
1. Three years experience clinical nursing experience required, critical care experience preferred.
2. Previous experience in Quality Resource Management desired.
3. Ability to prioritize responsibilities and use appropriate time management skills.
Other Special Training/Skills:
1. Must be familiar with community and state agencies, program, and resource service and their appropriate utilization.
2. Excellent communication and negotiation skills are essential.
Licensure, Certification, Registration, etc.:
1. Current license as a Registered Nurse in the State of Louisiana.
2. National certification desired
