RN Case Manager - All Boroughs Available

 RN Case Manager (Positions available in ALL Boroughs) 

General Purpose/Scope: 
The UAS Assessment RN is a Registered Nurse qualified in the assessment of potential and current members utilizing the UAS Assessment Tool. Coordinates and manages schedule to accomplish ADEQUATE number of UAS assessments in a timely manner while complying with MLTC Policies. Assesses potential enrollees and current members appropriately to justify MLTC enrollment. 
Essential Functions /Responsibilities: 
Conducts face to face assessment of members and potential enrollees utilizing the UAS Assessment Tool for functional status, medical, behavioral, psychological, and community resource needs 
Facilitate assessment of potential enrollees / members as per MLTC policies 
Competently assesses potential enrollees / members' community health need status and makes follow up suggestions as appropriate 
Collaborates with members of Care Management team as needed to assist in development of Participant's Person-Centered Service Plan 
Clarifies MLTC benefits, policies and procedures for potential enrollees, and current members, providers, and community based agencies 
Adhere to documentation policies and procedures 
Provide back up to other assessment nurses as needed 
Utilizes UAS analyzer for improvement of assessment and documentation skills 
Informs potential enrollees of services via network providers and provision of member's handbook 
Determines member eligibility for MLTC program and qualification for CDPAS 
Ability to perform other duties as assigned 
Skills/Aptitudes: 
Strong professional knowledge and comprehensive clinical assessment skills in frail and elderly population with chronic diseases 
Excellent communication skills 
Strong computer skills: Microsoft Office Products, (Outlook, Words, Excel) and Care Compass 
Ability to work independently, and be flexible in fast paced environment 
Ability to organize and manage multiply priorities 

Minimum Qualifications: 
NYS license and current registration as a registered nurse; 
Minimum of one to two years of experience in clinical practice; 
Minimum of one to two years of experience working with UAS assessment tool. 
Minimum of one to two years of experience working in MLTC, homecare, and or programs serving the frail and elderly (i.e., mobility, communication cognitive, or other impairments) 

Additional Preferred Qualifications 
Experience in managed care and/or care management 
Bachelor's or Master's degree in nursing 
Care Manager Certification
