ProMedica Hospice formerlyHeartland Hospice has an exciting opportunity for a full-time week day RN Case Manager (very limited on call after hours required, every 5th weekend and 1 holiday/year) to join our established growing hospice agency caring for patients in the Green Bay and surrounding areas. We have a openings available to include supporting patients within Green Bay and areas north. As a RN Case Manager, you will supervise and coordinate interdisciplinary care delivery to our home bound hospice patients and provide clinical visits throughout the weekdays. In addition, you will possess excellent written and verbal communication skills, good problem solving and decision-making skills and at least 6 months institutional nursing experience. Hospice experience is not needed as will train the right candidate.
