Position Overview
Are you passionate about providing high-quality, patient-centered care to older adults? We are seeking a compassionate and experienced Site Physician to join a collaborative interdisciplinary team within a PACE (Program of All-Inclusive Care for the Elderly) setting. This is an excellent opportunity for a physician who enjoys building long-term relationships with patients while leading clinical care in a supportive and team-oriented environment.
As the Site Physician, you will oversee the medical care of participants, collaborate with nurse practitioners and other healthcare professionals, and play a key role in improving clinical outcomes through coordinated, comprehensive care.
What You'll Do
- Serve as the primary care physician for an assigned panel of participants.
- Provide comprehensive medical care in both the day treatment center and participants' homes as needed.
- Lead and support an interdisciplinary team to develop individualized care plans.
- Collaborate closely with nurse practitioners and community-based providers.
- Participate in patient assessments, care planning, case conferences, and family meetings.
- Monitor quality of care and identify opportunities for continuous improvement.
- Assist with utilization review and quality assurance initiatives.
- Build relationships with specialists and community physicians to coordinate participant care.
- Mentor and educate clinical staff, trainees, and students.
- Support preventive care initiatives, immunization programs, and community outreach efforts.
- Participate as an active member of the interdisciplinary care team.
Qualifications
Required
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school.
- Board Certified in Internal Medicine or Family Medicine.
- Completion of an Internal Medicine or Family Medicine residency.
- Active, unrestricted New York State physician license.
- Medicaid and Medicare Provider IDs in good standing.
- Excellent communication and interpersonal skills.
- Minimum of one year of experience caring for frail or elderly adults, or willingness to complete specialized PACE training upon hire.
Preferred
- Board certification or additional experience in Geriatric Medicine.
- Fellowship or experience in Geriatrics or Palliative Care.
- Experience working within interdisciplinary healthcare teams.

- Monday-Friday
- 8:00 AM - 5:00 PM
- 40 hours per week
