Social Worker

The Social Worker assists with implementation of patient continuity of care by arranging discharge plans. In addition, the Social Worker provides supportive counseling to the patient, family and significant other. Education/Experience: Bachelor''s degree in a related field of Social Service with a minimum of two years of experience in Social Service, preferably in a medical setting, and passed the Indiana Health Professions Bureau Licensing Examination.
