Summary
According to social work practice and professional standards, works as a member of the healthcare team to facilitate continuity of care by assessing, planning, coordinating and implementing discharge planning appropriate to the patient's level of care and post-discharge needs. Provides supportive counseling, crisis intervention, financial assistance, and information and referral services to both internal and external customers.
Minimum Qualifications
- Master's of Social Work degree (MSW).
- Social Work license in the state of Pennsylvania or license eligible; must receive license within six months of hire.
- Strong verbal and written communication skills.
- Strong negotiation skills and the ability to address complex issues with patient/family and staff across multiple departments.
- Ability to work independently and to gather pertinent patient information.
Preferred Qualifications
- Minimum of 1-2 years experience in an acute care hospital setting.
- Fundamental understanding of personal computers and familiarity with the Windows operating system.
