Social Worker/LMSW

Conducting an initial assessment to understand the individual's strengths,
needs, and goals for community integration
Developing a person-centered plan that outlines the support needed to help
the individual engage more fully in their community and improve their overall
quality of life. Providing counseling services to help individuals manage mental health
conditions, behavioral challenges, or other issues that may hinder community
involvement.
Life skills training, such as social skills, communication skills, and
problem-solving techniques, to help individuals engage effectively in community
settings.
Connecting individuals with local community resources, such as vocational
programs, recreational activities, educational opportunities, or social groups.
Encouraging participation in community events and activities, fostering
relationships, and helping individuals navigate their environments in a way that
promotes independence.
Facilitating access to vocational training or supported employment opportunities
that help individuals enter or re-enter the workforce.
Advocating on behalf of individuals to ensure they have access to the services,
programs, and support they need in the community.
Supporting family members or caregivers with information and resources to help
them provide effective care and encouragement.
Developing and implementing crisis intervention strategies when needed,
ensuring that individuals have the support they need to stay safe and continue
progressing.
Referring clients to other specialized services as needed, such as mental health
treatment, addiction services, or specialized rehabilitation.
Maintaining thorough records of client progress, goals, and service plans to
ensure compliance with New York State regulations.
