Job Description:
- Contractor's employees shall be technically proficient in the skills necessary to fulfill the government's requirements, including the ability to speak, understand, read, and write English fluently as required.
- Contractor shall provide documents upon request to verify current and ongoing competency, refuse skills, certification and/or licensure related to the provision of care, treatment and/or services performed.
- The Contractor shall provide verifiable evidence of all educational and training experiences including any gaps in educational history for all contractor's employees and contractor's employees shall be responsible for abiding by the Facility's Medical Staff By-Laws, rules, and regulations (referenced herein) that govern medical staff behavior.
- Position Descriptions:
Social Worker:
- The services of Social Work shall be provided in accordance with the policies of the VA Healthcare System where assigned. Social Workers shall have professional and legal responsibility to the patient. Functions include but are not limited to:
- Perform Biopsychosocial Assessments.
- Assesses patient and plan appropriately to assist with social determinants of health
- Suicide Risk, Mental Health, and Crisis Assessment and intervention.
- Document safety plan and other plans of care and care given in patient's medical record.
- Provide information and completion of Advanced Directives.
- Assess and complete process for Petition of Inpatient Psychiatric Care.
- Communicates pertinent patient data to the providers and interdisciplinary team.
- Re-assesses patient's condition and revises plans-of-care based on problems that the Social Worker identifies.
- Provides resources and assistance with transportation.
- Schedule family meetings, interdisciplinary team meetings, or other goals of care conversations.
- Conduct goals of care conversations.
- Provide resources when the veterans have been determined to be homeless.
- Assess and address any findings of abuse or neglect. Consult Client as appropriate.
- Assess and assist with discharge planning which includes referrals to subacute
- rehabilitation, assisted living, long-term care facilities, inpatient rehabilitation, group homes, hospice/palliative care, and/or long-term acute care hospital.
- Assist with discharge planning for veterans who need resources when going home, such as home health consults.
- Able to initiate, prioritize, and reorganizes appropriate measures in emergency situations.
- Identify veterans who will need case management services post discharge.
- Assesses patient/family knowledge level, implements plan of instructions, and records the patient's family's demonstrated level of understanding.
- Can initiate life saving measures in the absence of Physicians.
- Assures the content and correctness of a prepared report and signs the document to validate its content.
- Maintains a neat personal appearance and maintains professional decorum.
- Consistently applies the VA's customer service standards, always demonstrating helpful courteous, and respectful conduct.
- Respects patient's rights, including but not limited to visual and auditory privacy.
- Reviews all information in the electronic medical records pertinent for social work services.
Requirements:
- Social Workers must have a minimum of an LCSW, two years' current experience in this field, and graduate from a nationally accredited school with an MSW.
