Trach/Vent Skilled Nursing Facility | Up to 30 Hours per Week

Community Physical Therapy (CPT) is seeking a PRN Speech-Language Pathologist to join our therapist-led team at Alden Park Strathmoor in Rockford, IL. This is a great opportunity to work in a skilled nursing environment supporting patients with complex medical needs, including trach and ventilator care.

Role Responsibilities
• Provide speech-language therapy services in a skilled nursing facility
• Evaluate and treat patients with swallowing, speech, and cognitive-communication disorders
• Collaborate with interdisciplinary teams to support optimal patient outcomes
• Build strong relationships with patients, families, and facility staff

Qualifications
• Licensed Speech-Language Pathologist
• Skilled Nursing Facility experience preferred
• Trach/vent experience preferred but not required
• Strong communication and teamwork skills
