Speech Language Pathologist
Fifth Star Care Management is seeking a dedicated and compassionate Speech Language Pathologist (SLP) to join our IN-HOUSE therapy team! New Grads Welcome!
Responsibilities
- Evaluate, diagnose, and develop individualized treatment plans for residents with speech, language, cognitive-communication, and swallowing disorders.
- Deliver evidence-based therapy services in accordance with physician orders and facility policies.
- Collaborate closely with an interdisciplinary team, including Physical Therapists, Occupational Therapists, Nurses, and Physicians, to ensure comprehensive care.
- Educate patients, families, and caregivers on treatment goals, home programs, and progress.
- Maintain accurate and timely clinical documentation in compliance with state, federal, and facility guidelines.
- Participate in care plan meetings and contribute to discharge planning as needed.
Qualifications
- Master's Degree in Speech-Language Pathology from an accredited program.
- Current State Licensure (or eligibility for licensure) as a Speech-Language Pathologist.
- Current Certificate of Clinical Competence (CCC-SLP) from ASHA (CFY candidates welcome to apply).
- Prior experience in a Skilled Nursing Facility or similar setting preferred, but not required.
- Strong interpersonal, organizational, and communication skills.
