Speech Language Pathologist Home Care
Job Summary
We are seeking a compassionate and motivated Speech Language Pathologist (SLP) to join our Home Care team on a part-time basis. The SLP will provide speech, language, cognitive, and swallowing evaluations and treatments to patients in their homes, helping them improve communication, safety, and overall quality of life.
Responsibilities
- Evaluate patients with speech, language, cognitive, voice, and swallowing disorders
- Develop individualized treatment plans based on patient needs and physician orders
- Provide skilled speech therapy services in the patient's home environment
- Educate patients, families, and caregivers on treatment techniques and home exercise programs
- Maintain accurate and timely clinical documentation
- Collaborate with physicians, nurses, therapists, and interdisciplinary team members
- Monitor patient progress and adjust treatment plans as needed
- Comply with all agency policies, professional standards, and regulatory requirements
Qualifications
- Master's Degree in Speech Language Pathology
- Current state licensure as a Speech Language Pathologist
- Certificate of Clinical Competence (CCC-SLP)
- Current CPR certification preferred
- Previous home health or geriatric experience is a plus
- Excellent communication and interpersonal skills
- Reliable transportation and valid driver's license
