Job Summary:
The Employee is responsible for the screening and assessment of patients at Centennial Medical Center to determine the patient’s appropriateness for post-acute placement. Employee provides assistance to patients, families, and staff as needed in the transition to post-acute care.
You will support the facility’s Standards of Performance and ICARE and I PROMISE values
Arranges and provides in-services for Case Management staff and other audiences as indicated.
Ensures that CMC staff receives appropriate and timely follow-up communications regarding their patients and identified post-acute needs.
Provides written reports to director or designee as indicated.
Analyzes patient condition, status, needs, and medical record data for cost/benefit analysis and medical determination for appropriate post-acute placement.
Effectively communicates post-acute evaluation, where indicated, and analysis to medical staff determining admission.
Accurately applies primary and secondary medical diagnostic information to support admission and treatment.
Completes admission assessment forms in a complete, legible manner after the choice letter has been signed and makes referral as indicated..
Verifies that Medicare days are valid.

Qualifications
Registered Nurse, Registered Physical Therapist or Registered Occupational Therapist from an accredited school with current licensure in the state of Tennessee.
Direct post acute experience, one year or more, involving extensive knowledge of post acute services and required criteria.
Excellent interpersonal skills/communication skills.
Excellent organization/time management skills.
Excellent customer service skills; professional appearance.
Experience in providing educational programs and public speaking relating to Marketing responsibilities.
